What to know about Form W-9
- A business or person who pays you asks for this form. You give the completed form to them, not to the IRS.
- Line 1 is your name as shown on your income tax return. Use line 2 only if you have a business or disregarded-entity name that is different.
- Check one box for your federal tax classification, such as individual or sole proprietor, or LLC.
- Part I asks for either your Social Security number or your Employer Identification Number.
- Sign and date Part II. A payer can start backup withholding if you don't give a correct taxpayer ID number.
Have these ready
- Your legal name and address
- Your Social Security number or EIN, whichever the form asks for
- The name of whoever requested the form, so it goes to the right person
